Session Description: The 1999 Institute of Medicine report “To Err Is Human: Building a Safer Health System” highlighted the need to make patient safety a major priority for healthcare authorities. This session describes a comparative analysis of the 5S tool with the five successive systemic barriers currently preventing healthcare from becoming an ulltrasafe industrial system. An application and three outcomes will be presented.
